Why would I need to change page size?

Most commonly because a document was made for a paper size your country or your printer does not default to — US Letter versus A4 is the classic mismatch between North America and most of the rest of the world. Converting beforehand avoids a printer awkwardly shrinking or cropping the page to fit.

Does this distort the content?

Very slightly, if the source and target proportions differ — Letter (8.5 × 11 in) and A4 (210 × 297 mm) are close but not identical in shape, so fitting one exactly into the other stretches it a small amount in one direction. It is not usually visible at a glance, and it is the trade-off every tool that does this exact conversion makes, rather than leaving empty margin instead.
